... and using telemedicine for patient visit
If you have a visit scheduled with us your appointment will still happen at the same time.
We will contact you to help set up telemedicine, which we are using due to COVID-19 concerns.
Telemedicine will replace all non-essential in-office visits with sleep medicine providers.
Please email (jenny@completesleepsolutions.com) a picture (front and back) of
your insurance card and identification (drivers license) prior to your
appointment.
Please complete the Sleep Questionaire Form prior to
your scheduled visit.

distance. It's a live interaction between a provider and patient, providing a
virtual alternative to an in-person office visit.
Who can use telemedicine? Anyone who:
has an existing appointment
need a new patient visit or follow-up visit
has a reliable internet/phone connection
has an iPhone/Android, laptop, computer, or iPad with working camera
